Poor Scarring – Patients who have undergone plastic surgical procedures have unsatisfactory scarring. There are a number of different ways in which scars can be suboptimal. Sometimes, scars stretch out or become excessively widened; sometimes, scars become darker than desirable (particularly in patients who have more pigmentation in their skin). On other occasions, scars can become thicker, raised and red which is a condition known as “hypertrophic scarring.” In certain individuals, the scar actually begins to grow large in size than the original incision, a condition termed a “keloid scar.” All of these scar abnormalities can be treated with modern plastic surgery techniques. Nonsurgical treatments include application of various creams and ointments like Biocornium which is a scar reduction gel or Eldoquin Forte (hydroquinone) which can be used to help bleach scars that are too dark. The application of sheets of silicone rubber may also help to flatten scars that are beginning to become excessively prominent. Injection of the scars with steroids such as Kenalog can also help to improve the appearance of thickened, elevated and excessively red scars.
When scars have been become abnormally wide or in patients who have extreme hypertrophy of the scars or keloid formation, surgical treatment is often warranted. Significant improvement can be achieved by excising wide scars and using careful surgical techniques to close the resulting defect. In patients who have large hypertrophic scars or keloid scars, excision of the scar followed by closure of the defect and then a short course of postoperative low dose superficial radiation therapy can be very effective in reducing recurrence of a deformity.
